Community Stroke Rehabilitation Team | St. Joseph's Health Care London

Our team of health care professionals deliver specialized services and coordinated care to individuals in the community who have suffered a stroke. Services are provided in the patient’s home or other community setting, through in-person and virtual care sessions. Our team includes a registered nurse, physiotherapist, occupational therapist, social worker, speech language pathologist, therapeutic recreational specialist and rehabilitation assistants.
